Pharmacists will inspect every donation to ensure it is:

• Cancer Medication and/or Supportive Cancer Medication

• Unopened and sealed in original manufacturer packaging

• Packaged with a visible lot number

• Not expiring in less than 6 months

• Room temperature storage

• Accompanied by Donation Form with lines 1, 3 (name of medication), and 6 completed for each medication donated

Not eligible for donation: opened, expired, refrigerated product, controlled substances and REMS program products.
